Transitions Canada provides day support services, out of home day respite, and supported independent living to adults that have a developmental disability. We also provide an after school and summer program when requested.
We provide supports that meet the person’s changing needs, wants, and goals and increase opportunities to live the life they choose within their community.
Our Passport eligible fee for service program is open to individuals 18 years of age or older.
Participants in our Abilities Centre are welcome for any number of days according to their needs from Monday to Friday. We nurture a variety of social and life skills and provide fun activities to practice the skills. Social skill development, personal growth, physical and mental health are the foundation of the program. We also provide supports according to individual goals, whether working toward employment, further education, or a change in living arrangement. We work with all skill levels. We try to operate in a scent free environment and appreciate the use of unscented products.
Our Supports for Independent Living assist the person to develop the skills to live safely and as independently as possible within their own home.
Respite is provided within our Abilities Centre and can be a full or half day; the person is able to join in on daily activities.
